Lawrence O'Donnell publicly acknowledged he was wrong to report a story not yet confirmed. Even though he said "if true" it was still unprofessional. That doesn't imply story was right or wrong, just not confirmed. That is what a mature responsible person does when he/she makes a mistake. In fact, Trump has repeatedly threatened to sue media figures, including O'Donnell, for stories unflattering to him and never does, because he would have to testify under oath and can't because the unflattering stories were correct.

No, that’s what a person does when they get caught lying. Mature responsible people correct themselves under their own recognizance, through some type of self discovery. Not because they’re getting sued. I will give some credit where it’s due, however. He didn’t double down on nonsense and he admitted his mistake, so fine. If Rachel Maddow had the decency to do the same I’d really be impressed. Maybe this winter she will at least refrain from telling people in North Dakota that Putin is gonna shut their heat off and freeze them to death.
Can't call it a failure right now. You can only call it an unverified statement. Releasing the tax and loan documents should clear it up so I expect trumps people to make sure they get it cleared up. Funny how trump gets all bent out of shape over this, but never apologized to the Clintons that he claims were responsible for Epstiens death with absolutely nothing to substantiate that. Seems besides a liar, trump is also a hypocrite.
I think it had something to do with Trump's tax return showing that he paid very little taxes. Or it may have had something to do with Trump's bank record showing that Russian Oligarchs had supported his losing business deals.
Could you please clarify where the failure was? Was it that the facts were wrong or was it that the source was not confirmed by another reliable source as news reporters should do?
I believe it was the latter. In order to know if the claims were false or not, Trump would have to actually release the tax returns...
