Are there a lot of contract problems in the NFL?

Lately I've noticed some articles pertaining to players and their contracts. It seems they're often complaining about not receiving extensions or not willing to play for the amount they are going to be paid.

Is this a common trend, or am I mistaken?

Who's fault is it usually? Player's or team's?

To me it always seems like the players are being selfish. But right now with my favorite team, the Titans, their star running back and best player Chris Johnson who just broke the all-purpose yards for a season record last season is set to get paid $550,000. That is second last of starting running backs and last on the starting 11 on offense. He is asking for 30-40 million and they should give it to him.

I think part is that most contracts are not guaranteed. I know Crayton for the Cowboys is worried Dallas will drop him instead of paying him 2 million his contract is for this season. He's worried they will drop him to late to get picked up by another team.
