Yep...that's it. Its a cache issue. To solve the problem hold down your shift key and click on refresh. It should work after that.
One of two things is happening here...either the pages are being cached or you are uploading the wrong avatar. I physically deleted the old avatar...
Try it one more time...and make sure you are uploading the correct picture.
