Basically my version of the fan is a stand that you can put your 360 on and it cools it (the 360 would be standing vertically). You only really need it if you got one of the early 360 releases which had a bunch of cooling/overheating problems. I bought mine because my console would freeze up a few times when I first bought it (this past summer) and a friend of mine recommended the fan. So, I bought it and haven't had any such problems since.

I've never needed a fan, but I guess it's a good idea and it's not expensive.I just picked up Splinter Cell: Double Agent as well. Awesome game so far.

Mine was about $10 at Best Buy... a lot of the more expensive ones aren't worth it, so just be careful.
